Caring for an aging loved one can quickly become complicated. There may be multiple physicians, medications, hospital visits, changing care needs, family members in different locations, and important decisions that need to be made, often with very little time.
West Coast Senior Solutions provides experienced care management and advocacy to help families coordinate those moving pieces and make sure their loved one’s needs remain at the center of the plan.
We can step in when a family needs ongoing oversight or simply needs an experienced advocate during a particularly difficult time. This may include attending medical appointments, communicating with healthcare providers, reviewing care needs, coordinating services, helping families understand recommendations, monitoring changes in condition, and identifying concerns that may require additional attention.
For adult children who live out of town, care management can provide another set of experienced eyes and ears locally. For families who are nearby but overwhelmed by the healthcare system, we can help organize information, ask the right questions, and make sure everyone involved understands the plan.
Advocacy When It Matters
A medical appointment, hospitalization, rehabilitation stay, or change in condition can leave families trying to absorb an enormous amount of information at once. We help families prepare questions, understand what is being recommended, communicate concerns, and advocate for their loved ones’ needs.
Our role is not to replace physicians, nurses, therapists, attorneys or other professionals. Our role is to help families navigate those systems effectively and make sure important information doesn’t get lost between them.
Ongoing Care Management
Some situations require more than a single consultation. When appropriate, West Coast Senior Solutions can remain involved to help monitor the care plan, coordinate services, communicate with family members and providers, and reassess needs as circumstances change.
Because aging rarely follows a perfectly predictable path, the right plan today may need to change tomorrow.
We help families recognize those changes and respond before a manageable concern becomes a crisis.
You Don’t Have to Manage Everything Alone
Whether your loved one is living independently, receiving help at home, transitioning from a hospital or rehabilitation facility, or living in a senior community, West Coast Senior Solutions can provide oversight, advocacy, and guidance for your family needs.
Our goal is simple: better communication, better coordination, and better-informed decisions.
